I've used "district medical officer" and called "zones de sante" health districts but this client uses "health zones" and I don't know whether to still use "district" in this case or whether there is something better.

The document is obviously related to the field of health and medicine in Sub-Saharan Africa.

Definitely continue to use "zones".
Firstly, it is hard going against the client's terminology unless you have a good argument and you don't have that here, not a clear grammatical error for example.
Secondly, you have nothing to justify "district" which is usually a smaller area and with things like a local municipality, post-code, etc.
A zone could well encompass many "districts" !!!

Chief Medical Officer (CMO) for the district/area (OR zone or region)

using "chief may or may not be needed as a CMO is sometimes in charge of an entire country or just an area or hospital so MO on its own may be all that's required

We don't use "zone " very often in English so try to persuade client but if they insist, leave them to it.
The word "zone" is used in this report and you may find more uses of it just like "health post" is used in a lot of areas rather than health "centre"
