1. Introduction
All Environmental and safety EHS incidents and accidents and emergency situations must be investigated to establish the cause, or causes, of such an event and to use that information to assist in preventing similar events in occurring. Effective investigations can help to alleviate the human pain and suffering, loss, and can help to prevent property damage. The investigation should cover Health aspects or issues where this is practical and efficient.
Health aspects need to be included were this is practical and efficient. By using “EHS” or “H&S” in this procedure we indicate that health aspects can play a role in the concerned management area.
2. Definitions
The Scope of this procedure is restricted to InBev Supply (production units primary logistic units).
Occupational accident: a discrete occurrence in the course of work which leads to physical or mental harm.
Occupational incident: a discrete occurrence in the course of work which could have lead to physical or mental harm but were such harm did not occur.
Included in occupational accidents for this procedure are:
All occupational accidents of the workforce and contractor employees working on premises of InBev supply units resulting, in non-fatal injuries with an incapacity for work of at least one day, excluding the day of the accident and fatal injuries.
Accidents outside the unit’s premises but sustained in the course of one’s work (e.g. road accidents) are included, but commuting accidents (see for the definition of commuting accidents e.g. Kalido S11) are excluded.
Occupational illnesses.
Occurrences having only a medical origin (such as a heart attack at work) are excluded.
The workforce includes normal contracts, temporary workers, seasonal and part-time employees.
Environmental accident: a discrete occurrence whereby a spill or release of a (e.g. hazardous, toxic or organic) substance that adversely impacts the environment happened.
Environmental incident: a discrete occurrence that could have lead to a spill or release of a (e.g. hazardous, toxic or organic) substance that adversely impacts the environment but were such a spill or release did not occur.
Severe Environmental Release or Spill: a spill or release of a (e.g. hazardous, toxic or organic) substance that adversely impacts the environment and has lead to media involvement or community complaints.
3. Policies
3a Incident and accident investigations: general requirements.
I. Units need to register and investigate every Occupational and Environmental accident.
II. As a minimum units also need to register and investigate the following Occupational and Environmental incidents:
o Occupational incidents that involve: forklift trucks, packaging equipment (e.g. palletisers and de-palletisers), lock-out tag out, electrical equipment or hazardous substances.
o Environmental incidents that could have lead to a Severe Environmental Release or Spill.
o Emergency situations that could have lead to Occupational or Environmental accidents.
III. Every unit must analyse investigate the Occupational and Environmental incidents and accidents mentioned in chapter I and emergency situations in a structured and efficient way, included the ones with contractors.
o If such an incident or accident or emergency situation takes place, the investigation must start as soon as possible and at least within 48 hours.
o Each unit plant does the investigation on basis of a method or methods defined by its zone organization and reports the findings of the incident, accident and emergency situation investigation to its zone organization according the zones instructions. If the zone organization has not prescribed any investigation method the method as defined in Annex 1 of this procedure has to be used.

o Mandatory reporting within 24 hours to the CTO, Global Director ES, Corporate Social Responsibility Director, Zone Presidents and BU Vice-Presidents, Zone ES Manager and the Country Crisis Manager Coordinator:
each deadly occupational accident (including contractors);
A spill or release of a hazardous or toxic substance that may adversely impact the environment beyond facility boundaries or may be potentially injurious to the public.
III. The head of the department where an accident or incident as mentioned in chapter I happens is responsible for the correct execution of the investigation the investigation of incidents / and accidents / emergency situations in his / her department, included the ones with contractors. The head of department does and the follow-up of the investigation findings in accordance with the contents and timing of the action plan that is made.
IV. A Copy of the investigation report must be sent to the unit’s ES manager. The ES manager reviews all incoming EHS incident and accident reporting in order to make sure that to have an overview of the overall correct working of the investigation process is executed correctly, the right conclusions are drawn and the right (corrective) actions have been identified implementation of all the findings and corrective actions if necessary.
V. The investigation and the follow-up of the action plan must be presented in the Safety Committee and in the Weekly Departmental Meeting of the department where the investigated accident or incident took place and the Monthly Plant Meeting.
VI. Visualization:
In each department/area, a visual board must be installed to indicate the place of the incident or accident or emergency situation. During the shift handover meeting, the occurred incident or accident or emergency situation must be discussed and improvement suggestions made must be treated via a PDCA cycle.
At the entrance of the unit plant, a visual board must be installed indicating the number of days without a Lost Time Occupational H&S lost time accident for the workforce own employees and for contractor employees working on premises of the unit.

VII. In order to make an exchange of learnings from incident and accident investigations between units possible the unit’s ES manager will send a copy of investigations that contain interesting learnings for other units to their Zone BS ES department. The Zone BS ES department will then make sure that the information is spread to concerned units in their zone. Zone organizations are allowed to set different requirements for the exchange of incident and accident learnings.
3b Safety / Environmental Alerts
A Safety / Environmental Alert is an urgent warning of the organization about a safety hazard or environmental risk. The Alert is usually based on learnings from an accident or incident that recently happened.
o The Alert will be drawn up by the Zone BS ES department in the zone were the event occurred.
o The Alert consists of a short description of the incident or accident, a description of the main causes, the preventive actions taken by the unit and recommendations of the Global and Zone ES organizations for the units on basis of which immediate mandatory action needs to be taken. See annex 2: VPO.QUAL.3.2.5.1.2 Safety / Environmental Alert format.
o GBS ES will spread the Alert to other Zone BS ES Departments.
o The ES Zone BS ES departments will spread the Alert to the Vice President Supply, Brewery Operation Directors and Plant Managers in their zone.
o GBS will publish the Alert on Livelink.
o The Plant Managers are responsible for the correct execution of the recommendations given in the Alert.
o The Brewery Operations Directors are responsible for the follow-up of the execution of the recommendations in the plants.
o The Zone BS ES departments will provide expert support to plants that need help with the execution of the recommendations given in the Alert.
3c Additional requirements in case of a fatal Occupational accident or a Severe Environmental Release or Spill
In case of:
o a fatal occupational accident of an InBev employee or a contractor working on one of InBev’s supply units or
o or a severe environmental spill or release on the premises of InBev’s supply units,
the following additional and mandatory requirements are applicable:
A. A first report of the accident must be send to the CTO, the Zone Vice President Supply; the Director Global ES, the VP Corporate Affairs, the Zone ES Manager and the Country CMT Coordinator. This action must be completed within 24 hours after the accident.
B. Within five working days after the accident a complete accident investigation in English must be send to officers mentioned in section A of this chapter.
C. The concerned ZBS ES will perform an independent investigation of the accident that has to be completed at the latest within 15 working days after the accident. This investigation will involve an on site investigation of the accident (e.g. in the involved plant).
D. As soon as possible but certainly within ten working days after the accident the concerned ZBS ES and GBS ES will spread a Safety / Environmental Alert.
E. If is established that the fatal Occupational accident or Severe Environmental Spill or Release is caused or is partly caused by a gross negligence of an InBev employee the organization will undertake proper disciplinary action against the concerned employee.
F. Each fatal Occupational accident or Severe Environmental Spill or Release must be discussed in the EBM board.

1. Uvod
Svi EHS incidenti i nezgode, vezani za okoliš i sigurnost i slučajevi opasnosti, moraju se istražiti kako bi se ustanovili uzrok ili uzroci jednog takvog događaja i iskoristiti takve informacije kao pomoć u spriječavanju nastajanja sličnih događaja. Efikasne istrage mogu pomoći u ublažavanju ljudske boli i patnje, gubitka, te pomoći u spriječavanju nastajanja štete na imovini. Istraga bi trebala pokriti zdravstvene aspekte ili pitanja gdje je to praktično i efikasno.
Treba uključiti zdravstvene aspekte tamo gdje je to praktično i efikasno. Uporabom “EHS” ili “H&S” procedura, dajemo do znanja da zdravstveni aspekt igra ulogu u referentnom području upravljanja.
2. Definicije
Svrha ove procedure je ograničena na ponudu InBev-a (proizvodne jedinice primarne jedinice logistike).

II. Kao minimum, jedinice također trebaju prijaviti i istražiti slijedeće profesionalne i nezgode iz zaštite okoliša i sigurnosti na radu:

III. Svaka jedinica mora analizirati i istražiti profesionalne nezgode i incidente i nezgode iz zaštite okoliša i sigurnosti , kao i nezgode spomenute u poglavlju I i slučajeve opasnosti, na strukturirani i efikasni način, uključujući i one sa izvođačima.
o Ako se dogodi jedan takav incident, nezgoda ili slučaj opasnosti, istraga mora početi što je prije moguće i barem unutar 48 sati..
o Svaki poslovni pogon provodi istragu na bazi metode ili metoda koje je odredila područna organizacija i o rezultatima istrage incidenta, nezgode i slučaja opasnosti informira svoju područnu organizaciju sukladno uputama područne organizacije. Ako područna organizacija nije propisala bilo koju metodu istrage koristi se metoda opisana u Dodatku 1 ove procedure.
o Vidi proceduru izvještavanja o nezgodama putem brzog skeniranja nezgode: VPO.QUAL. 3.2.12.1.
o Unutar 24 sata, obavezno je izvijestiti Izvršnog tehnološkog direktora, Globalnog direktora za zaštitu okoliša i sigurnost, Direktora korporacije za socijalnu odgovornost tvrtke, Područnog predsjednika i BU Podpredsjednika, Područnog ES menadžera te Državnog koordinatora za upravljanje kriznim situacijama.
Svaku po život opasnu nezgodu (uključujući izvođače);
Izlijevanje ili ispuštanje opasnih ili otrovnih supstanci koje bi mogle neprijateljski utjecati na okoliš izvan granica pogona ili bi mogli biti potencijalno opasni za javnost..
III. Voditelj odjela gdje se događa incident ili nezgoda iz poglavlja I je odgovoran za provedbu istrage istrage incidenata / i nezgoda / slučajeva opasnosti u njezinoj / njegovom odjelu, uključujući one sa izvođačima. Voditelj odjela provodi i prati odvijanje istrage i njene rezultate u skladu sa sadržajem i vremenom definiranim u akcijskom planu istrage
IV. Kopija Izvještaja o istrazi se mora poslati jedinici ES menadžera. ES menadžer pregledava sve dolazne izvještaje o EHS incidentima i nezgodama, kako bi imao pregled u kojoj mjeri se pravilno provodi istražni proces , donose ispravne odluke i definiraju ispravne (korektivne) akcije, primjena svih nalaza istrage i korektivne akcije ako je potrebno.
V. Istraga i provedba akcijskog plana se mora prezentirati Odboru za sigurnost i na tjednom sastanku odjela u kojem se dogodio odnosni incident ili nezgoda, kao i na mjesečnom sastanku pogona.
VI. Vizualizacija:
U svakom odjelu/području, treba postaviti vizualnu ploču, kako bi se naznačilo mjesto incidenta ili nezgode ili slučaja opasnosti . Za vrijeme sastanka o predaji smjene, moraju se razmatrati nastali incidenti, nezgode ili slučajvi opasnosti te prijedlozi poboljšanja koji se obrađuju putem PDCA (planiranje, djelovanje, kontrola, akcija) ciklusa.
Na ulazu u pogonsku jedinicu, postavlja se vizualna ploča koja označava broj dana bez prekida ili zastoja posla, a zbog izgubljenog vremena na osnovu profesionalnih nezgoda i H&S nezgoda ukupno zaposlenih, vlastitih zaposlenika i zaposlenika izvođača koji rade u poslovnim jedinicama.

VII. Kako bi se razmijenila znanja i informacije dobivene istragama o incidentima i nezgodama između poslovnih jedinica , ES manager će poslati kopiju istraga koje sadržavaju te informacije drugim jedinicama u njihove područne BS odjele za zaštitu okoliša i sigurnost na radu Područni BS Odjel za zaštitu okoliša i sigurnost na radu će onda poduzeti sve da se informacije dostave svim zainteresiranim jedinicama u području. Područnim organizacijama je dozvoljeno utvrđivanje različitih zahtjeva za izmjenu informacija i znanja o nezgodama.
A Safety / Environmental Alert je hitno upozorenje koje obajvljuje poslovna organizacija, a tiče se hazardne opasnosti ili rizika za okoliš. Upozorenje se obično temelji na informacijama i znanju koje se steklo iz nekog nedavnog incidenta ili nezgode.
o Upozorenje će objaviti područni BS odjel za zaštitu okoliša i sigurnost na radu i u području gdje se događaj dogodio
o Upozorenje se sastoji od kratkog opisa incidenta ili nezgode, opis glavnih uzroka, preventivnih mjera koje je poduzela tvrtka i preporuke globalnih i područnih organizacija
jedinicama koje akcije treba obavezno poduzeti. Vidi Dodatak 2: Format sigurnosnog upozorenja
o GBS za zaštitu okoliša i sigurnost će distribuirati Upozorenje drugim BS odjelima.
o Područni BS odjeli za zaštitu okoliša i sigurnost na radu će distribuirati Upozorenje Potpredsjedniku odjela nabave, Operativnom direktoru pivovare i pogonskim menadžerima u njihovom području.
o GBS će izdati Upozorenje na Livelink.
o Pogonski menadžeri su odgovorni za ispravnu realizaciju preporuka iz Upozorenja.
o Operativni direktori pivovare su odgovorni za realizaciju preporuka u pogonima .
o Područni BS odjeli za zaštitu okoliša i sigurnost na radu će pružiti stručnu podršku pogonima koji će trebati pomoć oko provedbe preporuka iz Upozorenja.
3c Dodatni zahtjevi u slučaju kobne profesionalne nezgode ili ozbiljnog izlijevanja ili ispuštanja
U slučaju:
o Kobne profesionalne nezgode zaposlenika InBev-a ili izvođača koji radi na jednom od opskrbnih jedinica InBev-a, ili
o Ozbiljnog izlijevanja ili ispuštanja u poslovnim objektima opskrbnih jedinica InBev-a,
primijenju se slijedeći dodatni i obavezni zahtjevi :
A. Prvi izvještaj o nezgodi se šalje Izvršnom tehnološkom direktoru, područnom Potpredsjedniku nabave; Globalnom direktoru za zaštitu okoliša i sigurnost na radu, Potpredsjedniku za odnose u korporaciji i Državnom koordinatoru za upravljanje kriznim situacijama. Ova akcija se mora obaviti unutar 24 sata od nezgode.
B. Unutar pet radnih dana nakon nezgode, šalje se dužnosnicima navedenim u odjeljku A ovog poglavlja potpuna istraga o nezgodi na engleskom jeziku.

C. Odgovorni ZBS ES će sprovesti nezavisnu istragu nezgode koja mora biti završena najkasnije unutar 15 dana od nezgode. Ova istraga će uključivati istragu na licu mjesta (na primjer u pogonu gdje se dogodila nezgoda).
D. Što prije moguće, ali svakako unutar deset radnih dana nakon nezgode, odgovorni ZBS ES i GBS ES će distribuirati Safety / Environmental Alert.
E. Ako je utvrđeno da je profesionalna nezgoda ili ozbiljno izlijevanje ili ispuštanje u okoliš dijelom prouzročeno velikim nemarom zaposlenika InBev-a, tvrtka će poduzeti potrebne disciplinske mjere protiv odgovornog zaposlenika.
F. Svaka kobna profesionalna nezgoda ili ozbiljno izlijevanje ili ispuštanje moraju biti razmotreni na sastanku EBM (EVIDENCE BASED MEDICINE), medicina na temelju dokaza.

REALIZZAZIONE DEL PONTE!
Inizialmente il dentista preparerà i denti che sosteranno il ponte riducendone la superfice esterna, creando così lo spazio per le capsule che li ricopriranno. Nel fratempo si applica un ponte provvisorio in resina acrilica. Questo, oltre a proteggere i denti preparati, consente anche di abituarsi al cambiamento. Quando il ponte definitivo è pronto, il dentista lo prova, effettuando eventuali ritocchi. Una volta che voi e il vostro dentista sarete soddisfatti del lavoro realizzato , il ponte sarà fissato con cemento apposito.
Con le appropriate cure i ponti e corone durano anni. La pulizia dentale quotidiana e uso del filo interdentaleè importante per la salute dei vostri denti e gengive. Nello stesso tempo è estremamente importante per le realizzazioni protetiche perché mantiene così la durata nel tempo.
CORONE
Quando un dente è daneggiato la corona dentale è una delle possibilità più efficaci. La corona può essere costruita in ceramica oppure in metal ceramica, e la scelta dei materiali dipenderà dalla situazione concreta.
Metal ceramiche corone sono le più resistenti, per la loro base metalica, ma effetto estetico migliore si ottiene con la realizzazione della corona ceramica.
La parte metalica della corona segue il movimento e la forma del dente, mentre la parte ceramica imita la forma naturale, colore e resistenza. Questa parte può essere realizzata come parte integrale del ponte oppure a parte.
COSTRUZIONE DELLA CORONA
La costruzione della corona si svolge in due fasi. Inizialmente il dentista preparerà il dente e prende un impronta che serve per fornire un modello del dente preparato. Da questo modello la corona verrà costruita in laboratorio, tenendo conto delle istruzioni del dentista. Nel tratempo si applica una corona provvisoria. Quando la corona è pronta, il dentista la prova, effettuando eventuali ritocchi. Una volta che voi e il vostro dentista sarete soddisfatti del lavoro realizzato , la corona sarà fissata.
IMPIANTI
Servono per la sostituzione di un dente mancante, dando al contempo la sensazione che si tratta di un dente vostro. Proprio per questo motivo l'implantologia è una delle soluzioni migliori. L'impianto può sostituire un solo dente oppure più denti mancanti. L'implantologia è usata sopratutto nei casi dove è necessario ricostruire e conservare l'anatomia dell'osso e gengive delle aree edentule.
Molti pazienti non riescono ad abituarsi ad una protesi. Le realizzazioni fisse protetiche sui impianti risolvono efficacemente tali problemi.
L'impianto contiene un radice artificiale e viene eseguito un foro nell'osso per posizionare l'impianto prescelto. Quando finisce il processo di ossificazione si costruisce il supplemento e poi la corona o il ponte.
Ad ogni modo, l'impianto migliora la qualità della vita.
Oggi giorno gli impianti non sono più una novità, ne tanto meno sono ritenuti tecniche sperimentali. I primi inizi gli scopriamo nei anni '50 in Svezia, ma solo nei anni '80 e '90 inizia largo uso dei impianti. Oggi, la riuscita dei impianti è valutata 90-95%.
Il vantaggio dei impianti consiste nella salvaguardia dell'osso e dei denti vicini, i quali non hanno bisogno di essere toccati, a differenza della protesi, la quale col tempo usura l'osso e diventa sempre più instabile e scomoda.

Postizanje prirodnog izgleda i osmijeha poslije gubitka zuba moguće je izradom mosta. Izradom mostova popunjava se praznina u zubnom nizu nastala gubitkom zuba.
Što je most?
Most se sastoji od barem dvije zubne krune sa svake strane praznine i međučlana. Most može biti na prirodnim zubima, na implantatima ili na kombinaciji: zubi i implantat.
Izbor vašeg mosta ovisit će o stanju vaših zubi, o financijskim mogućnostima, te o vašim estetskim željama.
Izrada mosta!
Stomatolog će u prvoj posjeti brusiti zubne nosače, te uzeti nekoliko preciznih otisaka koji se šalju u laboratorij i služe za izradu zubnih krunica i mostova. Pacijent iz ordinacije odlazi sa privremenim krunicama. Kada je most ili krunica gotov, pacijent ponovno dolazi u ordinaciju gdje se definitivan rad stavlja u usta, te ako je sve u redu rad se cementira, odnosno trajno fiksira.
Uz određenu brigu mostovi ili krunice trajat će godinama. Svakodnevno pranje zubi i čišćenje zubnim koncem važno je za zdravlje vlastitih zubi i gingive, a istodobno je izuzetno važno i za protetski rad jer se time produžava vijek njegova trajanja .
KRUNICE
Za zube koji su jako oštećeni, zubna krunica je vrlo efektivno rješenje. Ona može biti napravljena od čiste keramike ili metal-keramike, a izbor će ovisiti o konkretnoj situaciji. Krunica napravljena od metal-keramike je najčvršća varijanta radi metalne podloge, dok se estetski moment još bolje postiže čistom keramičkom krunicom.
Svojim metalnim dijelom krunica prati oblik zuba, dok keramikom imitira njegov prirodan oblik, boju i čvrstoću. Ona može biti zasebna ili kao dio mosta.
Izrada krunice!
Izrada ide u dvije faze. U prvoj posjeti stomatolog brusi zub te uzima otiske koji se šalju u laboratorij, a pacijent iz ordinacije izlazi s privremenom krunicom. U drugoj posjeti definitivna krunica je gotova, te se isprobava i cementira.
IMPLANTATI
Riječ je o nadoknadi izgubljenog zuba kod koje imate osjećaj da se radi o vašem prirodnom zubu. Upravo iz ovog razloga implantati su jedno od najboljih rješenja nadoknade izgubljenih zuba. Oni mogu zamijeniti samo jedan zub ili više njih, a koristimo ih i u slučajevima gdje je potrebno stabilizirati i retinirati zubnu protezu.
Mnogi pacijenti, iz različitih razloga, teško se navikavaju na nošenje proteze. Fiksni protetski radovi na implantatima rješavaju probleme nošenja proteze.
Implantat se sastoji od umjetnog korijena koji se usađuje u kost. Nakon što se usadi i nakon što prođe vrijeme potrebno za okoštavanje implantata, izrađuje se nadogradnja i zubna krunica ili most.
U svakom slučaju implantati dovode do poboljšanja kvalitete života.
Implantati danas nisu više novost, niti se smatraju eksperimentalnim. Začetke implantologije nalazimo još 1950-tih godina u Švedskoj, a tek 1980-tih i 1990-tih počinje njihova šira primjena. Danas je uspjeh u održanju implantata 90 - 95 % .
Prednosti implantata ogledaju se u tome što oni stoje samostalno, bez brušenja okolnih zdravih zuba. Implantati, osim čuvanja okolnih zdravih zuba, pridonose očuvanju kosti. Ni jedna druga metoda nema ovakve prednosti, jer na primjer, nošenje zubne proteze ubrzava gubitak kosti radi čega je ona vremenom sve nestabilnija i neugodnija za nošenje.

Članak 1.

Članak 2.
