Myopathy /rhabdomyolysis
Simvastatin and other HMG CoA reductase inhibitors occasionally trigger myopathy which becomes manifest as muscle pain, muscle weakness or muscle oversensitivity together with severely increased creatine kinase (CK, up to ten times the upper normal limit). Myopathies can sometimes occur in the form of rhabdomyolysis, with or without acute renal failure due to myoglobinuria. In addition, cases of death occurred rarely. The risk of myopathy is increased by high plasma levels of the inhibitory activity of the HMG CoA reductase.
The risk of myopathy/rhabdomyolysis is increased when using simvastatin concomitantly with the following medications
Potent CYP3A4 inhibitors: cyclosporin, itraconazole, ketoconazole, erythromycin, clarithromycin, HIV protease inhibitors or nefazodone, particularly with high doses of simvastatin (see CYP3A4 interactions under section ‘Interaction with other medicinal products and other forms of interaction’).
Lipid-lowering medication that may cause myopathy if given on its own: gemfibrozil, other fibrates or lipid-lowering doses of niacin ( ≥1 g/day), particularly with high doses of simvastatin (see interactions with lipid-lowering medication that may cause myopathy if given on its own under section ‘Interaction with other medicinal products and other forms of interaction’).

الأخرى أحيانًا يفجر (يثير) الاعْتِلالٌ العَضَلِيّ الّذي يصبح ظاهرا كألم العضلة، ضعف العضلة أو زياده حساسيه العضلة بالأضافه الي كينازُ الكرياتِين المتزايد بشدّة (كينازُ الكرياتِين ، حتّى عشرة أضعاف الحدّ العادي العلويّ ) . ّ
أحيانًا يمكن أن يحدث الاعْتِلالٌ العَضَلِيّ على شكل انْحِلاَلُ الرُّبَيْدَات ، مع أو بدون فشل كلويّ
حادّ بسبب بِيْلَةٌ مَيُوجْلوبينِيَّة (جلوبيولين عضلي في البول) .
بالإضافة لذلك، حالات الموت تحدث نادرًا .

يزيّد عند استخدام سيمفاستاتين ملازم ( مصاحب) بالمُداواة (بالأدويه )التّالية
موانع سيب3أ4 الفعّالة : سايكلوسبورين، إيتراكونازول، كيتوكونازول، الاريترومايسين،
كلاريثروميسين، موانع بروتياز فيروس نقص المناعة البشري أو نيفازودون،
تحت قسم خصوصًا مع الجرعات العالية لسيمفاستاتين (أنظر تفاعلات سيب3أ4
"التّفاعل مع المنتجات الطّبّيّة- الدوائية- الأخرى و أشكال التّفاعل الأخرى ").
المُداواة (الأدويه ) ٌ المخفضه للدهون الّذي قد تسبّب الاعْتِلالٌ العَضَلِيّ إذا أعطيت لوحدها :
جيمفيبروزيل، الفيبراتيز الآخر أوالجرعات المخفضه للدهون من النياسين
(≥1 جم/يوم)، خصوصا مع الجرعات العالية لسيمفاستاتين
الاعْتِلالٌ العَضَلِيّ إذا أعطيت (أنظر التّفاعلات مع الأدويه ٌ المخفضه للدهون الّذي قد تسبّب
و أشكال التّفاعل الأخرى "). لوحدها تحت قسم " التّفاعل مع المنتجات الطّبّيّة- الدوائية- الأخرى
