Questionnaire re Alzheimers
4. “Delirio de Abandono” (por ej: para ir a una residencia).
0 0 No se presenta.
1 0 Sospecha de que la persona que le cuida está conspirando para abandonarle o meterle en una residencia (por ej: por teléfono).
2 0 Acusa que hay conspiraciones para abandonarlo o meterlo en una residencia.
3 0 Acusa que van a dejarle solo de **manera inmediata** o a meterlo en una residencia.
For some reason I can't find much sense here. Will be immediately left alone? As opposed to being left alone at a later time???
Any help will be appreciated!

suddenly
Otra opción que me parece que puede quedar bien en este contexto, yo entiendo que la persona teme que la dejen sola o la internen "de repente, sin aviso"

immediately
Yes, will be left alone immediately, as opposed to being left alone at a later time.
Note from asker:
I still don't get it. How is it relevant within the context if it's immediate or later? Quiza me estoy ahogando en un vaso de agua... |

: This way makes sense to me, too.//I think the sympom is more acute if the person is afraid of being left NOW as opposed to some other time.

: Lydia, "de manera inmediata" sounds threatening because the person feels that he/she will be abandoned imminently. :)

on the spot
This is the Roget Thesaurus's best alternative for "immediately" in in this type of context. Examples with "abandoned":
She pretty much abandoned the idea on the spot, because when directed at any actual human being, it’s obviously ridiculous. Her easy capitulation was ...

This is about a delusion in which that the person believes he/she is going to be literally ABANDONED. It's not about being "left alone" or anything mild like that. That's why it needs the dramatic emphasis of "on the spot."
I couldn't find a "delusion of abandonment" in the DSM, but here is a similar condition that focuses more on efforts to avoid abandonment:
DSM IV Borderline Personality Disorder & Abandonment
The first criteria in the DSM IV for a diagnosis for borderline personality disorder (emotional regulation disorder) is:
"frantic efforts to avoid real or imagined abandonment. Note: Do not include suicidal or self-mutilating behavior covered in Criterion 5 "
Under "Diagnostic Features," the DSM IV says "The perception of impending separation or rejection, or the loss of external structure, can lead to profound changes in self-image, affect, cognition, and behavior. These individuals are very sensitive to environmental circumstances. They experience intense abandonment fears and inappropriate anger even when faced with a realistic time-limited separation or when there are unavoidable changes in plans (e.g., sudden despair in reaction to a clinician's announcing the end of the hour; panic or fury when someone important to them is just a few minutes late or must cancel an appointment). they may believe that this "abandonment" implies they are "bad." These abandonment fears are related to an intolerance of being alone and a need to have other people with them. Their frantic efforts to avoid abandonment may include impulsive actions such as self-mutilating or suicidal behaviors..."
