On a hospital form, before the ID number (teudat zehut) there is a field with samekh dalet under it (followed by 8 more fields for the ID number and another field with samekh bet - sifrat bikoret).

I assume the samekh stands for sifrat, but what does the dalet stand for?

Yes divided into 2 fields (for two digits)

Is the field above samekh dalet divided (like the ID which has 8 spaces etc.)or is it one field.

מספר סידורי

Can you not post in Hebrew text?
Anyway, it's probably "serial number" (I doubt the patient fills it in). I'm thinking of מס"ד which is מספר סידורי.
Desertfox may be correct but in this context I don't think you actually write "decimal number" in English.
