No context I'm afraid. Front page of "výpis z registra trestov". "Disclosure Print" (in a small frame/box) follows by the "Print of PNC Record" which I've translated as "výpis z registra trestov" (PNC - Police National Computer).
Any suggestions?

Ďakujem za doterajšie návrhy (zvlášť Radovanovi) Podľa jeho komentára/príspevku, ide o niečo ako "potvrdené tresty/napomenutia/výstrahy atď." Ako to ale "povedať" pekne?

detail. If there are no inhibitions against a party you can still print a “No
Disclosure Print”.
Note: – Once a Disclosure Print has been created you can no longer recall the
ROI search without incurring another fee. See page 11 On Recalling an ROI
search.

I googled both terms in quotation marks, as this seems to be an UK thing only and I found: The first screen of each record is the ‘nominal’ screen, which shows details of the subject’s name, date and place of birth, sex, ethnic code and last known address. According to Miss Lightley, after checking the nominal screen to see that the correct person had been identified, a ‘disclosure print’ would have been requested. It is important to note that a number
of other types of prints may also be requested from the PNC, including a ‘police print’. A ‘disclosure print’ only contains details of convictions, reprimands, warnings and cautions. It would not have included a charge which had been left to ‘lie on file’, as was the case on Huntley’s PNC record following the burglary (Contact 2).
So the phone call to your Local police would probably give you more details about this type of criminal record excerpt

Keďže britský PNC sa skladá z rôznych kategórií, ako je uvedené v tých opisoch, a možno žiadať o rôzne druhy výpisov z neho, teda nielen o informácie o trestoch, ale aj o iných veciach, napríklad o ešte neukončených policajných/súdnych záležitostiach, a rôzne iné policajné informácie, tak by som to za Register trestov nepovažoval a nazval by som to zatiaľ nejakým Policajným registrom.

No a toto by bol výpis z Policajného registra týkajúci sa len týchto ukončených vecí - keďže tento druh výpisu obsahuje - details of convictions, reprimands, warnings and cautions. It would not have included a charge which had been left to ‘lie on file’...

Je to dosť opisné, ale človek aspoň vie, čo to asi je.

Nemohol som ju použiť tak, ako to navrhuješ, pretože niektoré "cautions" boli iba od polície a nie súdu. Nakoniec som použil "Disclosure Print"/"Vypis z registra trestov" a "Print of PNC Record"/"Výpis z národného policajného registra". Možno nie najlepšie riešenie, ale klient bol spokojný. Vďaka."

If I am looking at it from US law point of view, disclosure is something which I am obligated to disclose by law to the other side, so this is my take on translation, put it into correct Slovak and check with the UK police, what is the exact meaning of the term in their content, but my educated guess is that I am right.

nie "zverejniteľne udaje" vo vyzname "verejne zverejnitelne", skus si pozriet 69 stranu, pol. 1.337 v uvedenej knihe

Skutočne by som sa tu vyhol výrazu "zverejniteľný", pretože každá strana sa začína poučením o dôvernosti ("strictly confidential").
