I was translating it as "no AMC". I think this one could have gone either way. It really depends on the context.

So, PMH for past medical history is generally accepted. However according to

The abbreviation can signify a number of things (there's a list) including alergias medicamentosas conocidas NKDA, as well as PMH.

Since antecedentes personales is often interpreted as past medical history, it would be redundant to use it twice. On the other hand, past medical history is initself redundant...

The list at the tremedica.org glossary is longer. Perhaps if you have a little more context you can narrow it down.
