Hello! I would very much appreciate your input regarding an adequate equivalent for this term. Here is the context:

"X jest uprawniony do składania Dostawcy Zamówień Dostawczych, zaś Dostawca jest zobowiązany do ich realizacji. Zamówienia Dostawcze będą zawierane, na zasadach określonych w Załączniku... w dniu podpisania Umowy oraz złożenia Zamówienia Dostawczego, w tym normami i przepisami wykonawczymi..."

I'm not sure if I should go for a more general Service Agreement/Commercial Contract option or focus on the delivery/supply angle...

Thank you both for your insight! It's surprisingly difficult to tell from the agreement whether the service offered is actually a physical product or a service as it appears both are covered! I don't believe I've ever encountered an agreement that is more vague -- but again, thank you for your invaluable help!

One more thought: the autor may see a difference between the supply of wares (physical products) and the supply of services (translation, cleaning, etc.) Therefore, two different order forms, etc.
This may be the reason behind "dostawcze" where trucks deliver things.

Zamówienie Dostawcze IMO an English-based Polish corpo bureaucratic newspeak (where each and every word needs to be translated literally).
A single word would suffice: zamówienie.
Just like anything to do with supply or serving as a vendor or provider, that's just one big vague thing, unless it miraculously has something to do with the nominate contract of supply from the Civil Code (i.e. make stuff and deliver stuff to customer) or some kind of EU newspeak that should be available on SIMAP. Also, it's not like Bubz isn't right. I just prefer to err on the less creative side. But it really looks like just a PO.

I'm actually quite relieved that this term doesn't have an obvious answer because I have literally spent hours researching it. The sketchy Polish definition I've gleaned from the internet has implied that it's a simple contract concluded between two parties for a service provided, but I know it has to be something else. Thanks for your help so far!

Supply Order

I'm rating it Low because, after all, it's the literal and functionally viable translation of a term that doesn't how an established translation. Still, 'zamówienie dostawcze' is some really ugly Polish. This is the first time I ever see that expression, and it is making me cry after all the machine-translated 'Polish' I saw when I tried Googling the term. The thing seems to be basically a PO except with supply in lieu of purchase.
