As I am a registered pharmacist, we use this type of records before dispensing more pharmaceuticals to patients to avoid drug-drug interactions among other issues.

Now, after posting the other 4 Kudoz, I am sure it is history record, since it will be needed during experiments or medication protocols to prevent:
1- drug-drug interaction with the experiment drug
2- patient allergies
3- keep a record for all drugs that patient has used, including the experiment drug.
