Spanruss is right; "estado" doesn't mean state (or government) here, it means estado civil, which is not limited to marital status. "Título" means a legal instrument establishing the status in question, in this case maternity. So "título de estado" means the instrument that proves status, but I think "proof of status" alone is sufficient.

I think this sentence has a structure that logically relates each clause to the next. I don't think it is best to translate the phrase "el título de estado" simply and literally. I suggest rephrasing the entire sentence to read: ". . . the title of mother is determined by the childbirth itself and is registered via a birth certificate in the Civil Registry."
