fiche de liaison – as in

Le fonctionnement du réseau s’articule autour d’une *fiche de liaison* qui doit être annotée par le pathologiste réalisant le diagnostic initial puis par le pathologiste du centre expert effectuant la double lecture.

It seems to be possibly a summary record card containing general patient details and details of diagnosis and treatment.

Is there any standard English term?

Hi thanks everyone for you input. I have decided against the two follow-up options as they seem directly related to follow-up care, and in the 2015 proz ref, to reminding patients of appointments. My apologies I should have given a little more context, this is to do with recommendations that specific conditions should always be assessed by two clinicians. Having carried out more research my impression is that there are different types of fiches de liaison, as Phil says here it is about liaison between professionals. So I in the end I went for liaison sheet - thanks Phil, did you want to post that as an answer?

A literal translation works perfectly well. There's no standard name - each institution uses different terms. It's used for liaison between different professionals.

Hi Phil - as I indicated this seems to be a standard document so I assumed it had a standard name in English-speaking hospitals.

@phil It's not what they use in English-speaking hospitals, + 'liaison' may have other connotations in English
