Each child has missed appointments but xxxxxx has 13 canceled appointments and 16 no shows. xxxx had failed her vision test on xxxx and she had not yet seen an optometrist. xxxxx is exhibiting aggressive behaviors towards his siblings and peers at xxxxx. xxxx was referred to mental health and a pediatric allergy appointment for xxx and she has missed both. Ms. xxxx herself has a long standing history of Depression, PTSD, and Anxiety without treatment.

STRENGTHS:
The xxxx family is open to receiving services that may be needed and acknowledge that they need to improve their communication and attendance for medical visits. The family has safe and stable housing and the parents are financially stable. The youngest child, xxx, is up to date and is doing well. The children are visible in the community.
