My pleasure Using voice recognition software without checking it can unfortunately happen in very pressurised situations but I think it is pretty bad practice when it isn't corrected and can be dangerous. The only other possibility is that the person who has made this record doesn't have a good command of Hebrew, which can also happen and also is not ideal.

That is a brilliant idea. I was pretty sure it was a spelling mistake, but I could not figure out any clues to work out what it's supposed to be... The amount of typing mistakes I have found on medical reports....

כמעט? The only thing I can think of is that כימת should possibly be כמעט. This could happen, for instance, if they are using voice recognition software when dictating the clinical notes and didn't realise it made a nonsense of what was being said. In that case it would mean something like "she is hardly ambulatory" or "she hardly moves at all" or something like that.
