Thanks to both of you for your input; in the end, I used "primary osteoarthritis", also known as "idiopathic o.a.".

I am removing my answer as I think the sense but not the terminology is correct. Primary centred osteoarthritis looks correct as far as I can see

arthrose Yes to osteoarthritis. At least, that's the correct translation of "artrosis" in Spanish (which I mostly translate), and I presume the same applies to "arthrose" in French. Not just "arthritis", which is used quite loosely in English, sometimes as shorthand for rheumatoid arthritis. I think the basic difference is that "arthrite" is inflammatory and "arthrose" is degenerative.

"Primary centred" sounds pretty convincing to me, but I'm afraid I don't understand what "essentielle" means here.
