Makes sense to me Splenomegaly would normally trigger an evaluation for relevant infections, and therefore the doctors, who were sure enough about the (non-infectious) etiology of this patient's splenomegaly, felt the need to state explicitly that such testing was not done.

In my case, this is associated with alcoholism and other chronic conditions of the patient, hence my initial query here.

splenomegaly ... is (to me, although I'm not a doctor) a symptom; you would not call it a disease. However, it might be caused by an infection. Just saw this online: "An enlarged spleen can be caused by infections, cirrhosis and other liver diseases, blood diseases characterized by abnormal blood cells, problems with the lymph system, or other conditions. Here are some common infections that can cause an enlarged spleen: Viral infections, such as mononucleosis." etc.

The Mayo clinic web site says "Many conditions — including infections, liver disease and some cancers — can cause an enlarged spleen, also known as splenomegaly"

It seems to me, then, that splenomegaly can be associated with infectious disease.

Joseph, thank you for your suggestion. I thought I was missing something completely, as splenomegaly is not an infectious disease. I might query this with my client, as it contradicts the rest of the medical report and despite any suggestions, seems out of place.

various meanings You sometimes can't just translate words directly -- you have to translate the meaning. In my glossary I have the following options for bilancio: assessment, analysis, evaluation. So if this is under the test results, it's saying that an infectious evaluation/assessment was not performed during the hospitalization.
