This is a sentence in a benefits guide. I can't figure out the above the & higher in this sentence.

I am not entirely sure, but the text appears to come from: http://www.iesonestop.com/wp-content/uploads/2018/02/IES-201... It appears to be badly edited. "1 If you go to a provider that is not in the xx Network, you will be responsible for paying: any billed charges above the Allowable Amount (explained on previous page), higher coinsurance and deductibles, the full amount of any limited or non-covered services, and a failure-to-preauthorize penalty if you do not get advance approval for a service." So, if "(explained on previous page)" was omitted, you are missing a comma, i.e. ".... Allowable Amount, higher coinsurance ....".
