"Affastellato il parenchima alle basi."
This comes from a chest x-ray - other findings are pleural effusion and cardiomegaly. I've found the explanation below, however I'm still struggling to find the precise terminology in English. "Il termine “affastellamento”, che secondo il dizionario Hoepli della lingua italiana indica l’azione e il risultato dell’affastellare, ossia raccogliere in fastelli, laddove il fastello è un grosso fascio di legna, come al solito non vuol dire nulla; o quantomeno non è possibile tradurlo in un’informazione clinicamente utile. [...] da questo punto di vista il “diffuso rinforzo del disegno bronco-vasale” potrebbe avere un suo perché, anche se potrebbe essere chiamato con un’altra e più corretta denominazione)."

If the basal lung regions are affected, the problem is often some sort of dystelectasis (supine film, insufficient or inefficient inspiratory effort, adipose patient, diaphragmatic palsy, compression by pleural effusions). This is often described as "consolidation".
Do you have any information on the diagnosis of this patient?

BTW: In your unradiologo source, the radiology professor is quoted as saying: "Poi schiaccia la scopa a terra: stai “affastellando” i fili della scopa, ossia li stai avvicinando tra loro, allo stesso modo in cui si affastellano i vasi polmonari quando un paziente respira poco e male."
This would support your and my idea of "consolidation", I think.

Hi Anne, thanks for your reply. I did think of "consolidation". This gentleman has a bit of everything in his patient history, however the main diagnosis is heart failure. |
