REPORTING OF COMMUNICABLE DISEASES
The clinician or other responsible health care worker should without delay notify the local health authority that a communicable or a peculiar disease exists within the particular jurisdiction. Administrative regulations that describe which communicable diseases are to be reported and how they should be reported may vary greatly from one region to another because of different conditions and different disease frequencies. This manual presents a basic reporting scheme. The purpose of disease reports is to provide necessary and timely information to permit the institution of appropriate investigation and control measures by responsible health authorities. In addition, the reporting scheme encourages uniformity in morbidity and mortality reporting so that data among different health jurisdictions within a country and among nations can be compared.

Discussion
Regarding الإبلاغ, "reporting" is the standard term. In some sentences, the verb "to notify" might be appropriate, but not in this context. More to the point, "surveillance" has no place here at all.
Regarding المعدية, you would be right to be somewhat hesitant, because there are at least four English terms, one of which has a significantly different meaning.
The first three terms are "communicable," "transmissible," and "infectious." They all mean pretty much the same thing. As a classifying term, "communicable" seems to predominate.
The fourth term is "contagious." All contagious diseases are communicable, but not all communicable diseases are contagious. We call a disease contagious when it spreads relatively easily or rapidly from one person to another (not through a vector, like flies or ticks).
As a public health regimen, reporting is required for all communicable diseases, not just contagious ones.
Finally, Yassine has a point, and I am sure he means very well, but this is not the place to discuss it. It is personal and very complicated.
First, it appears that you have difficulty with the word التبليغ and the word المعدية. You should, therefore, have posted two separate questions, one for each term, especially considering that the phrase التبليغ عن الامراض المعدية does not even exist in your text.
Second, I am glad you have now explained the nature of your difficulty with each term, but you should have done so when you posted your question. There is no need to wait until someone prompts you. We cannot help you if we don't know the nature of your difficulty, especially if the term itself is a standard, everyday term, like الإبلاغ and المعدية. We cannot guess what is going on in your mind.
