English to Russian Other Insurance Hospice Policies (USA)

I understand that by choosing the hospice, any care related to my terminal illness must be provided and/or authorized by Х to be covered by Medicare, Medicaid, or related insurance, and by doing so I waive any care related to my terminal illness that has not been authorized by X such as:
