This is a form to be filled in by employees before undergoing a test for COVID-19.
The brief section entitled 'Recueil du consentement' offers two options:
'Proposé: [] Oui [] Non'
This is followed by a standard statement beginning 'Je déclare donner mon consentement libre et éclairé pour la réalisation...'.
I am unsure how to understand this 'Proposé'. Does it mean the options 'offered' by the health authority or the consent 'granted' by the person filling in the form?
I think its superfluous nature is at the root of my understanding.
