Spanish to English Medical Medical: Health Care abbreviation
This appears at the bottom of a "patient status" report from Mexico. It's essentially where the signature line would be, but it has the title and no signature.

I think M is Medico and C is Cirujano and Gral, of course, is "general," but I am flummoxed by the A

It could also be this format: MÉDICO/A CIRUJANO GENERAL. If you copy and paste this text in quotation marks in google you will see a number of webpages using the exact phrase.

Aquí, seguido de "general", no parece tener mucho sentido lo de atención continuada. Aunque ni idea; sin otro contexto, y asumiendo la idea de cirugía general, médico ambulatorio, médico asistente, o incluso Manolo Arias, cirugía general.

medico de atencion continuada In Spain, at least, this is one meaning of MAC.
