1. Incident Description (per CPAR):
During running upper completion, tong die was heard dropping from die carrier whilst moving tong from well center to park position after last connection make-up. Tong die was observed between slips segments in rotary. Attempted to recover tong die, however position made retrieval difficult. Tong dies subsequently lost in hole.

2. Contributing Factors/Root Causes/Remarks/Notes:
 Preliminary Causal Factors were identified:
• General overall jaw condition appears poor with evidence of corrosion and cracking around die retention lugs. Jaws were at least 11 years old.
• Jaw condition was impaired.
3. Short Term/Immediate Actions:
 Management/Supervisors to share alert to all R&M and field personnel to reiterate that tong jaws are an integral component of all TRS packages and must be maintained according to their respective maintenance requirements in MyAdvisor PFTs and inspection required throughout the job execution.
