Czech to English Medical Medical: Pharmaceuticals
U těžkých stavů vyžadujících *útočnou dávku* se podávají 2-3 mg/kg tělesné hmotnosti/den rozděleně do 3-4 dávek.

Any ideas? I've never heard this before. The medicine concerned is prednisone

For example, a lifetime use of prednisone makes skin so thin that it brakes at the slightest touch. A "loading" dose of it is usually smaller, then adjusted. (This from knowing an asthmatic). It is not an antibiotic or a behavioural science drug.

Prednisone is a real killer on a prolonged basis. So even "emergency dose" IMO, could be used. Or, as Mr. K. says, emergency high dose

initial dose/loading dose

The first or first two doses that are stronger and in shorter time interval than the maintenance dose that follows.

Or even "exceptionally high dose". The literal translation, "shock dose", was used in medical literature many decades ago but not anymore.
Anna's variant, loading/initial dose, is one of the cases when a high dose is administered, but it does not match your context. Here, it's just a high dose warranted by the severity of the condition. For prednisone, the most typical condition requiring an exceptionally high dose is a circulatory shock, or possibly a situation when the patient in a critical condition needs to be transported for much longer than usual (e.g. rough terrain and no helicopter). Similarly, patients in prolonged severe pain (cancer, extensive burns, etc.) may require (and will easily tolerate) a dose of opiates that is otherwise considered lethal.
Source: my own past experience as a mountain rescue paramedic.
