This is under previous point, No.8, where the patient confirms that he has received clear information on the procedure etc. by checking a tick-boxes list for "written format" "multimedia technology" "meeting with consultant" etc.
...Then follows sentence No.9, followed by No.10 (as follows):

9) o di aver deciso di delegare a ………………………………… da me stesso autorizzato, la ricezione delle informazioni secondo la modalita al punto 8, desiderando non essere messo a conoscenza delle stesse

10) di essere a conoscenza della possibilita di REVOCARE il presente consenso in qualsiasi momento prima dell’intervento e di chiedere ogni informazione qualora lo desideri


Obviously I can translate this literally, but I do not understand the point of this sentence, so I may risk mistranslating the whole thing....!

Some patients do not wish to be told of medical information that regards them (for example if their health condition is serious or terminal) and prefer decisions to be handled by a relative or someone else. That's what this is referring to.

mettere a conoscenza : apprise of; be reported. There's a similar French turn of phrase with 'conscience'.

If the consent form is to be translated more legalistically than medically : being desirous of not being apprised (vs. the common UK tabloid press and police mis-spelling of appraised) of such.
Example sentence:

And I shouldn't feel I could... proceed unless you were fully apprised of... Mi sento di non dover... Procedere senza averti messo a conoscenza di...

Basically the undersigned delegates someone else to receive the medical info that concerns them on their behalf. For whatever reason, they do not wish to be informed personally.

He is saying in 8) that he wants to "receive" the information, but he just does not want it to be sent directly to him.

9) or that I have decided to delegate to ......................................., authorised by myself, the receipt of the information according to the modality of point 8, wishing not to be made aware of it

10) that I am aware of the possibility of WITHDRAWING this consent at any time prior to the intervention and to request any information if I so wish
