When deciding between easy and pro, err on the side of pro. Most questions will be pro.

* Note: non-member askers are not given the option of entering 'pro' questions; the only way for their questions to be classified as 'pro' is for a ProZ.com member or members to re-classify it.

I also think that the translation should be as literal and in as formal a register as possible.

Source text should have a colon as follows I think:

"Pour l'état civil : de l'extrait d'acte de naissance ou du livret de famille"

So who is issuing it and who is presenting it? I don't find this very clear.
What does "issued on production" mean? I still don't get it.
