After a brief search, I also came across the website shared by Ms. Shera Lyn Parpia and I clarified my doubt concerning the letter V in the acronym. I agree with her on the first part of her translation Part II Series B Form V, but I think U.1 is a reference to the department where the certificate is filed therefore I would translate it as Department 1/Dept.1.
