I think we have been digressing a bit. Mr. Vašek had asked a simple question to which the three of us each gave a simple answer, to our best knowledge. I gave an EU link which basically mirrors the Czech texts in many aspects. Other links were given. We got "sort of stuck" on the issue of what is goods and what is not, singular intended. Probably my fault. Anyway, may the deceased travel safely. :)

Article 4 of the 1973 Agreement And Article 2 of the 1937 Convention is linked to Article 4 of the 1973 Agreement: S výjimkou dokladů požadovaných na základě mezinárodních úmluv a dohod týkajících se všeobecně přepravy nebo na základě budoucích úmluv a ujednání o převozu těl zemřelých, nebude ani stát určení ani tranzitní stát požadovat žádné další dokumenty kromě průvodního listu těla zemřelého.

Given Mr Vašek’s description, it looks more like it could be the initial document required by Article 2 of the Convention in order for the průvodní list to be issued.
Státem určení nebo státem průjezdním nebudou požadovány, mimo obvyklé dopravní listiny předepsané mezinárodními úmluvami, týkajícími se přepravy vůbec, žádné jiné průkazy než umrlčí pas [průvodní list], jak stanoví předchozí článek. Tento pas nebude vydán odpovědným úřadem leč na předložení:
2. úředního potvrzení o tom, že proti přepravě není námitek z hlediska zdravotního nebo policejně – lékařského […]

Přeprava lidských pozůstatků, na kterou se vztahují mezinárodní smlouvy, jimiž je Česká republika vázána, se provádí způsobem upraveným těmito smlouvami. Průvodní list k přepravě lidských pozůstatků, potřebný podle příslušné mezinárodní smlouvy,12) vydává krajská hygienická stanice.

Are human remains goods? This is an interesting question. How else would customs treat human remains, if not as goods? HMRC refers to them as goods here https://www.gov.uk/hmrc-internal-manuals/export-procedures/e... , and several cases are mentioned here http://www.mcbreensenior.com/cases/86-2802.html . And it looks like the hygienická stanice is treating them as goods, too, given the four options on offer.
