This is probably two companies merging, and then immediately hiving off a part of their business to create another company.

-- the first date is when a "fusion-scission" occurred, when a part of this company was demerged / spun-off
-- 16 years later a company was merged with this company and the merged company kept the name of this company.
If it's a large(ish) company you could probably find on the Web public announcements / press releases about what happened at these dates (what kind of recombination happened at these dates).
Also, you could look at the whole entry in the Register of companies (usually the access is free), as details of any mergers / demergers must be part of the public record.
Then no more need for guessing or assumptions.

- that the company no longer exists as it has been merged to another company
- that another company has been added to it and the merged company has this name
and few other variations.
One way or another "fusion-scission" CAN NOT happen without at least one other company being involved, (or a change of ownership). Any other company name mentioned?
