Under the heading "Données Patient" I have the usual

CONTRASTE

This form is from the Centre Hospitalier de Hull (Canada)

Does anyone know what "IDP" stands for? the box is located under "Assurance" and after "Taille"
Unfortunately there is not information imputed in box.

Hi everyone,
Thanks so much for your assistance!
Unfortunately the IDP box is empty.
The line underneath has a redacted box followed by "SC M2", "POIDS (LB)", POIDS (KG)", "ALLERGIES CONNUES", "CONTRAST 1", "CONTRAST 2", TOTAL (ML), CLAIRANCE CREAT.

Then there is a second section with "ANALYSES BIOLOGIQUES" and a third section "HEURES EVENEMENT"

Could it be "Identifiant du Patient"? Stab in the dark here.

It would be interesting to know if there is any data following this. The fact that SC follows this could suggest that IDP is "INJECTION INTRA DERMIQUE PROFONDE" as opposed to "INJECTION SOUS CUTANEE". But I am only hazarding a guess obviously

Could it be "indice de poids"? The usual term for BMI is "indice de masse corporelle", but this often seems to be used as an alternative. The only thing is that it comes before "weight", and I'd expect it to come after.

Jenny Duthie has already suggested this, but given all the apparent uncertainty I am making the answer official. I agree also with Anne Schulz that SC is likely surface du corps.

These are essentially vital statistics and demographics.

The suggestion that it means "platelet distribution width" is clearly incorrect. That parameter is of minimal importance and would never be included with the type of data that was presented here.
