That's the precise meaning implied in this expression, at least in Buenos Aires.

Sí, son equivalentes. Sin embargo, "a pulmón" tiene algo más que el trabajo independiente o autofinanciado; denota un esfuerzo grande, una lucha. En el caso del texto original, ese es el sentido que tiene.
