I would like to know the best translation of this phrase from European Spanish into American English. It's the translation of some educational documents. Is okay to translate as file guidelines?

(Wrong answer.) The answer chosen, relating to the "proceedings (or actions)", is completely unheard-of in this context (an academic transcript or the like).

Alternatives which can be used instead of 'observations' may be 'remarks' or 'comments'. Possible translations of 'expediente' are 'expedient', 'means', 'actions', 'proceedings'.

Since this is about an academic record, observations refer to remarks and comments without actions or proceedings.

I'd definitely not choose the words proceedings or actions, as those are more used in a legal sense. I think that if it's a student's transcript where you're describing the subjects, the part of the "Observaciones del expediente" refers to any relevant comments in relation. However, maybe more context would be useful in order to identify if it's refering to what I mentioned before or to the fact that maybe in the presentation of the file/record there were some documents missing and that is the section where the clerk makes some notes about it.

Please note that there is no necessity to clarify there. "Observations" is OK in English since it has an anaphoric reference.

neutral Marcelo González : Could 'observations' alone NOT convey a slightly different (more inclusive, less restrictive) meaning? Absent more context, I'd include a clarifying term in this context, and 'on file' seems fine to me (though others may exist as well).

Here the word "records" may be modified depending on context. (More context from the Asker would be desirable. I assumed that the records are academic records of a student, such as a Transacript of Record.)
It is evident that the "Notes" are explanatory notes to aid the reader in reading the Records.
