agree  Simon Mountifield: Yes, often caused by a build-up of ice on the aircraft's wings (frequently mentioned in the Mayday / Air Crash Investigation series - try watching the episode "Pilot Betrayed"). Incidentally, mesh screens were considered, but ruled out as too dangerous..
