A 72-year-old female with triple vessel coronary artery
disease was referred for coronary artery bypass surgery.
She had a history of heparin exposure in coronary angiography
(CAG) one month before. She underwent OPCAB
with left internal thoracic artery (LITA) grafted to left
anterior descending branch (LAD) and the radial artery
(RA), anastomosed to right internal thoracic artery (RITA),
grafted to obtuse marginal (OM), posterolateral (PL) and
posterior descending (PD) branches sequentially.

A rotational atherectomy is performed and subsequently a non-drug eluting stent is placed in the LIMA. Via the RC vein bypass graft, the posterolateral has a nondrug-eluting stent placed.

Na 99,9% nie jest to placebo, a na 90% nie jest to też "posterolateral". Taka nazwa spotykana jest w odniesieniu do odgałęzień prawej tętnicy wieńcowej, ale żeby tam zlokalizowane zmiany powodowały niestabilna dławicę, a jeszcze żeby w tym miejscu wykonać PCI, to byłoby bardzo dziwne (aczkolwiek nie jestem specjalistą). Jest wprawdzie znikoma możliwość że chodzi o tętnicę okalającą (gałąź lewej t. wieńcowej), ale nie spotkałem się jeszcze z takim nazewnictwem. Najbardziej prawdopodobne jest jakieś przekłamanie/błąd w zapisie - już jest jeden błąd ewidentny: DS zamiast DES.

Rozwój badań nad chorobą niedokrwienną serca doprowadził do dużego zamieszania w nazewnictwie tej choroby. Ze względu na objawy kliniczne (ból w klatce piersiowej i duszność) choroba była nazywana dusznicą bolesną (ale też dławicą piersiową, łac. angina pectoris, gr. ankhone – duszenie, łac. pectus – klatka piersiowa), natomiast z uwagi na patofizjologię choroby nazywana jest chorobą wieńcową (łac. morbus coronarius) lub niewydolnością wieńcową (łac. insufficientia coronaria). Ponieważ choroba może przebiegać w postaci ostrej i przewlekłej, dodatkowo używano sformułowań: stabilna (odpowiadająca dzisiejszemu pojęciu przewlekłej choroby niedokrwiennej serca) i niestabilna (odpowiadająca dzisiejszemu pojęciu ostrego zespołu wieńcowego). W postaci niestabilnej dodatkowo rozróżniano pojęcia zawału serca (lub ściślej zawału mięśnia sercowego), zawału serca zagrażającego (zwanego niestabilną chorobą wieńcową). To zamieszanie zakończyło się z końcem XX wieku, kiedy to chorobę niedokrwienną zaczęto kwalifikować jako przewlekłą chorobę niedokrwienną serca oraz ostre zespoły wieńcowe.

Przezskórne interwencje wieńcowe (ang. percutaneous coronary interventions, PCI) – techniki inwazyjnego leczenia choroby wieńcowej ZA POMOCĄ NARZĘDZI WPROWADZANYCH przezskórnie do tętnic wieńcowych zwężonych przez proces chorobowy (najczęściej blaszkę miażdżycową) w celu ich poszerzenia lub udrożnienia (rewaskularyzacja) i przywrócenia prawidłowego krążenia (leczenie reperfuzyjne). Wedle aktualnych wytycznych jest to metoda z wyboru leczenia pacjentów z ostrym zawałem mięśnia sercowego
Zalicza się do nich między innymi:
• przezskórną (śródnaczyniową) angioplastykę wieńcową (ang. percutaneous transluminal coronary angioplasty, PTCA);
• aterektomię:
• kierunkową tnącą (ang. directional coronary atherectomy, DCA);
• rotacyjną (rotablacja);
• brachyterapię.

TORAKOTOMIA TYLNOBOCZMNA
Torakotomia – procedura chirurgiczna, polegająca na otwarciu ściany klatki piersiowej, umożliwiająca dostęp do narządów zlokalizowanych wewnątrz klatki piersiowej, czyli płuc, serca, przełyku, tchawicy, przepony.

Ten rodzaj zabiegu zapewnia dobrą kontrolę dróg oddechowych i jest często wykonywane u pacjentów z urazami klatki piersiowej. Ten rodzaj torakotomii jest też wykonywany jako tak zwana emergency thoracotomy, nawet w leczeniu przedszpitalnym[1] w przypadku penetrujących ran klatki piersiowej.[2]Wykonuje się ją w V międzyżebrzu z przodu, wykonując cięcie od mostka w kierunku pachy.[3] Umożliwia ona odbarczenie narastającej tamponady serca, czy bezpośredni masaż serca.
