It is a basic Czech template, available at any stationery shop, e.g. here https://www.sevt.cz/produkt/zprava-osetrujicimu-lekari-14024... I guess it is a hospital report to the GP. Jiri Lonsky Czech Republic Is the source correct? 10:10 Dec 8, 2022  It is quite unclear who (and why) would write a report to an attending physician. Standard English terms concerning this subject are "attending physician statement" or "attending physician's report", both written by the attending physician, often for insurer's purposes. Notes to answerer Asker: Díky za radu, kdo zprávu vystavil nevím (tedy myslím obor, vím jen jméno). Myslela jsem, si, že by mohl existovat nějaký pěkný termín. Formulář této zprávy je běžně dostupný. Asker: Díky! :) discharge mi tam chybělo, a to jsem také čerpala ze stránek VFN.
