This is from an MRI report. Initially, the word "nach" appeared superfluous, but then it appeared again. What do you think is meant in the following instances?

- Unauffällig belüftete miterfasste basale NNH und Mastoidzellen. *Nach* vereinzelter zervikaler Lymphknoten ohne Überschreiten des pathologischen Signifikanzniveaus exemplarisch im Level 1A mit einem Kurzachsendurchmesser von ca. 4 mm links.

- Unauffällige Darstellung der Larynx ohne Nachweis eines raumfordernden oder ausgeprägten entzündlichen Prozesses. Im paramedian linken Lobus pyramidalis *nach* einer ca. 6 x 7 mm messenden diskret kontrastmittelaufnehmenden ovalären Formation des Schilddrüsenparenchyms mit sonst isointensem Signalverhalten im Vergleich zum übrigen Schilddrüsenparenchym DD Schilddrüsenknoten.

The pyramidal lobe of thyroid (also known as Lalouette pyramid 5) is a normal anatomic variant representing a superior sliver of thyroid tissue arising from the thyroid isthmus. It is seen as a third thyroid lobe and is present in 10-30% of the population.

It represents a persistent remnant of the thyroglossal duct. It usually arises from the right or left side of the isthmus extending in a cranial direction;


so I can only find this in relation to "extending in a cranial direction"
