Whichever translation you choose, there's no need to use initial capitals just because the Spanish does. These are not proper nouns.

As I understand it, "concertada" means privately owned land that, for the purposes of a nature reserve, also receives public funding.

In UK English, “private” refers to “private persons/bodies”. In this particular case, town councils, i.e. public entities funded and administered by the local government, can also be the owners of such natural reserves.

I agree it's a bit long. Maybe put Coordinated nature reserves and then a footnote with the explanation. I think most Spanish understand the term as there are "colegios concertados" ' partly privately and partly publicly funded, and it's a common expression.

True, we are on the same line. But "Nature Reserves coordinated between the government and the landowners" is extremely long, and perhaps not very handy to be used as a translation. Again: The problem here is "concertado/-a", a very "particular" term conveying a very specific notion of cooperation, but bear in mind that "reserva natural concertada" is as difficult to grasp for Spaniards as its literal counterpart is for English speakers. The explanatory note is a must. And a final point to make clear is the distinction among the different types of natural/nature reserves.

I agree with Toni that the term "concerted" causes confusion and also that it needs some explanation. I think that a good alternative would be to say Nature Reserves coordinated between the government and the landowners. It gives better clarification of their difference from the others.
